Roles & Responsibilities

• Evaluate patients’ functional abilities and occupational performance to set meaningful, measurable goals
• Develop and implement individualized OT treatment plans focused on independence and safety
• Document all patient care and progress thoroughly using EPIC
• Collaborate with nurses, physicians, PTs, SLPs and other team members to coordinate patient-centered care
• Provide adaptive equipment recommendations and train patients and caregivers in safe use
• Participate in Trinity Specialty Specific Competency Testing and complete required skills checklists
• Maintain required health screenings, immunizations and fit testing to meet facility standards

Qualifications

• Active Occupational Therapy license (license/certificate verification required)
• BLS certification required
• Successful completion of Trinity Specialty Specific Competency Testing and Trinity Skills Checklist
• Ability to pass required health screenings and immunizations (drug screen, TB/PPD or IGRA, COVID-19 vaccine, MMR, Hep B, Tdap, influenza, respirator evaluation where applicable)
• Comfortable documenting in EPIC and working across multiple clinical sites within the counties served
• Strong communication and teamwork skills; patient-focused and adaptable

Knowledge & Skills

• Proficient documentation and charting in EPIC
• Skilled in OT assessment tools, activity analysis, ADL training and adaptive equipment recommendations
• Knowledge of infection control practices and ability to complete fit testing and respirator evaluations
• Competence with Trinity-specific competency processes and skills validation
• Excellent interpersonal skills for patient education and interdisciplinary collaboration
