Roles & Responsibilities
- Assess and evaluate speech, language, cognition, and swallowing function for SNF residents
- Develop individualized treatment plans and provide skilled therapy to improve communication and swallowing safety
- Document progress and outcomes clearly and timely in the resident record
- Collaborate with nursing, rehab team members, families, and caregivers to coordinate person-centered care
- Deliver evidence-based interventions that aim to maximize functional independence and quality of life
- Educate staff and families on strategies to support carryover of communication and swallowing recommendations
Qualifications
- Certificate of Clinical Competence (CCC) required
- Active SLP license in Florida (state licensure required)
- 1-3 years of clinical experience, ideally with skilled nursing facility (SNF) populations
- Experience with adult dysphagia and cognitive-communication disorders preferred
- Strong interpersonal and documentation skills
Knowledge & Skills
- Clinical proficiency in adult speech, language, cognitive-communication, and swallowing evaluation
- Ability to create clear, person-centered treatment plans and measurable goals
- Skilled at interdisciplinary collaboration and caregiver education
- Comfortable working in a skilled nursing/post-acute care environment
- Efficient documentation practices and timely clinical reporting
