Candidate Pay Rate: $70 per hour

Roles & Responsibilities
- Assess complex wounds and ostomies using evidence-based tools and clinical judgment
- Develop and implement individualized wound and ostomy care plans that improve outcomes
- Perform and supervise wound treatments including sharp debridement and Wound Vac therapy when indicated
- Provide expert ostomy care and education to patients and families to promote independence
- Collaborate with interdisciplinary teams including PT, respiratory, pharmacy and social services to coordinate patient-focused care
- Document thoroughly and accurately in Epic to ensure continuity of care and clear communication
- Conduct risk assessments such as Braden scale and ABI to guide prevention and treatment strategies
- Serve as a resource and educator for staff on infection control, isolation precautions and best practices in wound and ostomy management
Qualifications
- Active Registered Nurse (RN) license in the applicable state
- National Alliance of Wound Care and Ostomy Certification required (WOC cert)
- Basic Life Support (BLS) certification required
- WOCN credential preferred
- Minimum 1 year of relevant wound/ostomy experience
- Experience with Epic charting preferred
- Comfort working in a teaching and rehabilitation hospital environment
Knowledge & Skills
- Wound assessment and management including pressure ulcer staging and tunneling identification
- Ostomy care and management including ileostomy and colostomy care and equipment selection
- Competence with wound treatments and dressings including alginates, foams, hydrogels and silver products
- Experience with Vacuum Assisted Closure and wound vac therapy
- Sharp debridement and mechanical debridement skills
- Braden scale assessment and prevention risk assessment tools
- Ankle-Brachial Index (ABI) for peripheral arterial disease evaluation
- Infection control practices, isolation precautions and PPE use
- Strong patient and family teaching skills to promote self-care
- Familiarity with Epic electronic health record for documentation
