Roles & Responsibilities

• Evaluate and treat patients using medically prescribed physical therapy programs to restore function and minimize disability
• Develop individualized treatment plans, set meaningful goals, and implement skilled interventions
• Perform initial admission/evaluation visits and document clinical findings thoroughly
• Complete timely OASIS documentation and notify the Clinical Team Manager of scores within established timelines
• Plan and document discharge goals and progress to ensure continuity of care
• Supervise and mentor Physical Therapy Assistants when appropriate
• Coordinate care with the interdisciplinary team to optimize patient outcomes
• Maintain accurate, timely documentation of observations, treatments, and goals

Qualifications

• One year of Home Health experience (required)
• Experience performing admission/evaluation visits and OASIS documentation
• Experience or comfort supervising Physical Therapy Assistants
• Strong documentation skills and ability to communicate OASIS scores to clinical leadership
• Reliable transportation for territory/home visits across assigned areas

Education

• Degree in Physical Therapy (e.g., DPT or MPT) preferred

Knowledge & Skills

• Skilled in patient evaluation and treatment planning for home health populations
• Proficient with OASIS documentation and timely clinical reporting
• Comfortable delivering skilled interventions and developing discharge plans
• Experience with home health workflows, territory visits, and flexible scheduling
• Ability to supervise and collaborate with Physical Therapy Assistants and clinical team members
