Roles & Responsibilities

Evaluate patients and develop individualized treatment plans that drive recovery
Provide skilled therapy focused on mobility, strength, balance, and functional independence
Document patient progress and treatment outcomes accurately and in a timely manner
Collaborate with nurses, CNAs, therapists, and other clinicians to ensure coordinated care

Qualifications

Active California Physical Therapist license (required)
Experience in Skilled Nursing Facility (SNF) or similar settings preferred
Strong clinical reasoning and patient-centered treatment planning skills
Excellent documentation and communication skills
Ability to work independently and contribute as a collaborative team member

Knowledge & Skills

Proven expertise in physical therapy evaluation and treatment planning
Skilled in mobility, strength training, balance interventions, and functional training
Clear, timely clinical documentation and outcome tracking
Strong interpersonal skills for interdisciplinary collaboration
Comfort working in a fast-paced SNF rehab environment
