Contract Type: Per-Diem
Shift Type: Per diem (days vary), 6:00 AM - 6:30 PM Day shift (30-minute meal break)
Hours per Week: 36
Candidate Pay Rate: $33 per hour

Roles & Responsibilities
- Provide compassionate direct patient care to residents at Bear Canyon Rhb Ctr
- Administer medications and treatments accurately and on schedule
- Monitor, assess and document patient status and progress in the chart
- Communicate clearly with the interdisciplinary team and report changes
- Assist with admissions, discharges and smooth patient transitions
- Follow infection control and safety protocols to protect patients and staff
Qualifications
- Licensed Practical Nurse (LPN) — active and in good standing (required)
- Valid LPN license for New Mexico (or compact privileges if applicable)
- Clinical experience in long-term care, rehabilitation, or acute settings preferred
- Current BLS/CPR certification or willingness to obtain promptly
Education
- Graduation from an accredited Practical Nursing program and active LPN licensure
Knowledge & Skills
- Strong medication administration and documentation skills
- Patient assessment and basic clinical nursing procedures
- Effective communication and teamwork with clinical staff
- Comfortable working in a fast-paced rehab/long-term care environment
- Attention to detail and commitment to patient safety
