Roles & Responsibilities

• Provide compassionate direct patient care, including medication administration and basic nursing treatments
• Perform thorough patient assessments and monitor changes, escalating concerns to the care team
• Document nursing care accurately and timely in the patient record to ensure continuity
• Collaborate with nurses, physicians, and support staff to deliver coordinated, patient-centered care
• Assist with admissions, discharges, and safe patient transfers while maintaining dignity and safety
• Support infection control and safety practices to protect patients and team members

Qualifications

• Current Licensed Practical Nurse (LPN) credential — ready to practice (New Hampshire licensure preferred)
• Current BLS/CPR certification or willingness to obtain upon hire
• Clinical experience as an LPN preferred; new grads with solid training are encouraged to apply
• Strong communication skills and ability to work collaboratively in a fast-paced setting
• Ability to pass required background checks and meet facility onboarding requirements

Education

• Completion of an accredited LPN/Licensed Practical Nurse program

Knowledge & Skills

• Solid knowledge of medication administration and basic IV/PO procedures
• Patient assessment and vital sign monitoring skills
• Familiarity with wound care and basic dressing changes
• Comfortable with electronic health records and timely documentation
• Able to prioritize tasks, multitask safely, and maintain attention to detail
