Roles & Responsibilities

• Provide compassionate, hands-on care to residents and patients with dignity and respect
• Assist with activities of daily living (bathing, dressing, toileting, mobility) while promoting independence
• Take and record vital signs accurately and communicate changes to the care team
• Follow individualized care plans and document care clearly in the patient chart
• Support safe transfers and positioning, using proper body mechanics
• Collaborate with nurses and interdisciplinary team members to ensure excellent patient outcomes
• Respond promptly to patient needs and contribute to a calm, caring environment

Qualifications

• Active Licensed Nursing Assistant (LNA) credential — VT or NH licensure preferred given the regional assignment
• Comfortable working evening shifts (3:00 PM - 11:00 PM) on a per-diem basis
• Professional, dependable, and able to pass required background checks and health screenings
• Previous experience in long-term care or similar settings is a plus
• CPR/BLS certification is a plus (or willingness to obtain upon hire)

Education

• Completion of a state-approved LNA program or equivalent and high school diploma or GED

Knowledge & Skills

• Strong patient-care skills and hands-on experience with ADLs
• Ability to take and record vital signs accurately
• Basic charting and documentation skills
• Infection control and safe-handling practices
• Effective communication and teamwork with nurses, residents, and families
• Compassion, patience, and attention to detail
