Roles & Responsibilities

• Provide compassionate hands-on care to residents, assisting with daily living activities
• Monitor and record vital signs and patient status accurately and promptly
• Assist nursing staff with safe transfers, mobility, and comfort measures
• Follow established care plans and communicate changes in condition to the care team
• Maintain a clean, safe environment and adhere to infection control protocols
• Document care delivery and observations clearly in resident records
• Support mealtime, hygiene, and toileting needs with dignity and respect

Qualifications

• Current Licensed Nursing Assistant (LNA) certification/classification
• Eligible to work in VT or NH as indicated by regional assignment
• Basic life support (BLS/CPR) certification preferred or willingness to obtain
• Ability to pass background check and required health screenings
• Comfortable working overnight shifts and with per diem scheduling
• Strong communication skills and a team-oriented attitude

Education

• High school diploma or equivalent required
• Completion of an approved nursing assistant program and active LNA certification

Knowledge & Skills

• Comfortable performing ADLs and safe patient handling techniques
• Ability to take and record vital signs accurately
• Basic charting and documentation skills
• Knowledge of infection control and standard precautions
• Compassionate bedside manner and strong interpersonal skills
• Ability to follow care plans and escalate concerns appropriately
