Roles & Responsibilities
- Evaluate patients thoroughly to identify functional limitations and craft individualized treatment plans
- Design and implement rehabilitation programs that restore mobility, independence, and confidence
- Deliver evidence-based physical therapy interventions in a skilled nursing facility setting
- Collaborate with nurses, OT, SLP, physicians, and case managers to coordinate holistic care
- Document progress and treatment plans clearly and timely to support continuity of care
- Educate patients and families on home programs, safety, and strategies to maximize recovery
- Participate in care planning and discharge preparation to ensure safe transitions
- Maintain a safe therapeutic environment and adhere to facility policies and infection control practices
Qualifications
- Licensed Physical Therapist in Virginia or eligible for licensure (must obtain prior to start as required by facility)
- Doctor of Physical Therapy (DPT) preferred; open to PTs with equivalent credentials
- Any experience level considered — including new graduates (clinical readiness and enthusiasm matter!)
- Comfortable floating to nearby units/facilities as needed
- Strong communication and documentation skills to work within interdisciplinary teams
Education
- Doctor of Physical Therapy (DPT) preferred; PT degree or equivalent required
Knowledge & Skills
- Skilled in evaluation and treatment planning for geriatric and post-acute populations
- Experience or comfort working in a Skilled Nursing Facility environment
- Proficient with clinical documentation and electronic medical records
- Ability to safely mobilize and handle medically complex patients
- Strong interpersonal skills for coaching patients and collaborating with care teams
