Roles & Responsibilities

• Evaluate patients thoroughly to identify functional limitations and craft individualized treatment plans
• Design and implement rehabilitation programs that restore mobility, independence, and confidence
• Deliver evidence-based physical therapy interventions in a skilled nursing facility setting
• Collaborate with nurses, OT, SLP, physicians, and case managers to coordinate holistic care
• Document progress and treatment plans clearly and timely to support continuity of care
• Educate patients and families on home programs, safety, and strategies to maximize recovery
• Participate in care planning and discharge preparation to ensure safe transitions
• Maintain a safe therapeutic environment and adhere to facility policies and infection control practices

Qualifications

• Licensed Physical Therapist in Virginia or eligible for licensure (must obtain prior to start as required by facility)
• Doctor of Physical Therapy (DPT) preferred; open to PTs with equivalent credentials
• Any experience level considered — including new graduates (clinical readiness and enthusiasm matter!)
• Comfortable floating to nearby units/facilities as needed
• Strong communication and documentation skills to work within interdisciplinary teams

Education

• Doctor of Physical Therapy (DPT) preferred; PT degree or equivalent required

Knowledge & Skills

• Skilled in evaluation and treatment planning for geriatric and post-acute populations
• Experience or comfort working in a Skilled Nursing Facility environment
• Proficient with clinical documentation and electronic medical records
• Ability to safely mobilize and handle medically complex patients
• Strong interpersonal skills for coaching patients and collaborating with care teams
