Roles & Responsibilities

• Perform thorough care coordination to ensure safe, timely transitions from hospital to next level of care
• Lead discharge planning activities, creating clear, patient-centered plans of care
• Conduct utilization review using InterQual criteria to support appropriate level-of-care decisions
• Document thoroughly and accurately in Epic and other computerized charting systems
• Arrange post-acute services independently, including DME, SNF placements and ambulance transport
• Facilitate family meetings and participate in interdisciplinary treatment planning
• Collaborate with Social Services and Transportation to remove barriers to safe discharge
• Apply DRG/CMS knowledge to regulatory and documentation requirements

Qualifications

• Active California license required (pending licenses accepted but must be active at start date)
• Minimum 2 years acute care hospital case management experience performing care coordination and discharge planning (hospital experience is a must)
• Travel experience required (first-time travelers may be screened by clinical team but are accepted)
• Proficiency with Epic and computerized clinical documentation
• Comfort with weekend rotation and ability to meet department scheduling needs
• Experience arranging DME, SNF and ambulance transport independently
• Working knowledge of InterQual criteria, DRG concepts and CMS regulations

Knowledge & Skills

• InterQual Criteria and utilization review processes
• Care coordination and discharge planning best practices
• Epic and computerized documentation systems
• DRG and CMS regulatory awareness
• Familiarity with HMOs/IPAs and community resources
• Competence in arranging DME, SNF and ambulance transport
• Experience with patient populations including adolescents, adults and geriatrics
• Experience with behavioral health intersections (drug/alcohol treatment, patients experiencing homelessness, mental health/crisis) and family meeting facilitation
• Proficient in Microsoft Office
