Roles & Responsibilities

• Provide compassionate, hands-on nursing care to residents in a skilled nursing and rehab setting
• Perform timely patient assessments and communicate changes in condition to the care team
• Administer medications and treatments safely and accurately according to orders
• Document care thoroughly and maintain accurate clinical records
• Collaborate with nursing, therapy, and support staff to create individualized care plans
• Support resident mobility, rehabilitation goals, and activities of daily living
• Advocate for resident safety, dignity, and quality of life during every shift

Qualifications

• Current, active LPN or RN license (Licensed Practical Nurse or Registered Nurse)
• Clinical experience in long-term care or rehabilitation is a plus
• Comfortable with medication administration, wound care, and basic nursing procedures
• Strong communication and teamwork skills to coordinate with interdisciplinary staff
• CPR/BLS certification as required by the facility (or willing to obtain)

Education

• LPN: Graduate of an accredited practical nursing program
• RN: Graduate of an accredited ADN or BSN nursing program

Knowledge & Skills

• Assessment of vital signs and basic patient condition monitoring
• Medication administration and documentation best practices
• Basic wound care, ADL assistance, and mobility support
• Clear, timely clinical documentation and handoff communication
• Infection control principles and safe patient handling
• Empathy, patience, and strong interpersonal skills with residents and families
