Roles & Responsibilities

• Deliver safe, compassionate bedside care to residents, following care plans and physician orders
• Administer medications and document treatments accurately and timely
• Monitor residents’ conditions and communicate changes to the interdisciplinary team
• Assist with activities of daily living while promoting resident dignity and independence
• Support admissions, discharges, and transfers with clear, organized documentation
• Collaborate with nurses, therapists, and support staff to create a positive care environment
• Participate in quality improvement and infection control practices to keep everyone safe

Qualifications

• Active LPN license in Massachusetts (required); RN license also accepted where applicable
• Current CPR/BLS certification
• Minimum clinical experience in long-term care or rehabilitation preferred
• Strong interpersonal skills and the ability to work well in a team
• Ability to pass background checks and meet facility onboarding requirements

Education

• Graduation from an accredited LPN program (required)
• High school diploma or equivalent (minimum)

Knowledge & Skills

• Solid understanding of nursing fundamentals, medication administration, and documentation
• Familiarity with long-term care and rehabilitation workflows
• Good assessment and critical thinking skills to identify changes in resident condition
• Comfortable using electronic health records and other clinical documentation tools
• Compassionate communication with residents, families, and care teams
