Roles & Responsibilities

• Provide skilled, compassionate nursing care to residents and patients in a rehab and nursing setting
• Administer medications and treatments safely and document accurately
• Perform and document patient assessments, reporting changes in condition promptly
• Collaborate with interdisciplinary teams to support individualized care plans
• Assist with admissions, transfers, and discharges to ensure smooth transitions
• Support patients and families with education, reassurance, and clear communication
• Maintain a safe, clean environment and follow infection control and facility policies

Qualifications

• Current LPN license in Massachusetts (LPN) — RN candidates with appropriate license may also be considered
• Basic Life Support (BLS) certification required or willing to obtain
• Prior experience in long-term care, rehab, or skilled nursing preferred but new grads with clinical readiness are welcome
• Strong clinical judgment, reliable documentation skills, and a patient-first attitude
• Ability to pass background checks and meet facility onboarding requirements

Education

• Graduate of an accredited Practical Nursing program for LPN; ADN/BSN for RN candidates

Knowledge & Skills

• Medication administration and basic IV/route knowledge appropriate to scope of practice
• Wound care basics, restorative nursing, and mobilization assistance
• Accurate charting and electronic health record familiarity
• Effective communication with patients, families, and interdisciplinary teams
• Time management, prioritization, and the ability to handle a fast-paced clinical environment
