Roles & Responsibilities
- Assess patients' mobility, strength, balance, and functional needs to create personalized plans
- Develop and implement effective treatment plans using evidence-based interventions
- Provide therapeutic exercise, gait training, and functional re-training to maximize independence
- Document patient progress clearly and timely in the medical record
- Collaborate with nurses, aides, physicians, and family members to coordinate care
- Educate patients and caregivers on home programs and safe functional strategies
- Discharge planning coordination to ensure seamless transitions and appropriate follow-up
- Contribute to a positive practice culture by mentoring colleagues and sharing best practices
Qualifications
- Current, unrestricted Physical Therapist license in Florida
- Active Basic Life Support (BLS) certification (American Heart Association or equivalent) preferred
- Comfortable working day shifts and able to provide coverage any day of the week, including weekends
- Strong clinical assessment and documentation skills
- Excellent communication and teamwork skills with patients, families, and staff
- Prior experience in post-acute, rehab, or outpatient settings is a plus but open to motivated clinicians
Education
- Doctor of Physical Therapy (DPT) preferred, or equivalent degree in Physical Therapy
Knowledge & Skills
- Skilled in therapeutic exercise prescription, manual techniques, and functional mobility training
- Knowledge of common diagnoses seen in post-acute rehabilitation and appropriate interventions
- Proficient with electronic medical records and accurate clinical documentation
- Ability to prioritize caseloads, manage time effectively, and adapt to changing patient needs
- Strong patient education and caregiver training capabilities
- Good problem-solving skills and clinical reasoning
