Roles & Responsibilities

• Assess patients' mobility, strength, balance, and functional needs to create personalized plans
• Develop and implement effective treatment plans using evidence-based interventions
• Provide therapeutic exercise, gait training, and functional re-training to maximize independence
• Document patient progress clearly and timely in the medical record
• Collaborate with nurses, aides, physicians, and family members to coordinate care
• Educate patients and caregivers on home programs and safe functional strategies
• Discharge planning coordination to ensure seamless transitions and appropriate follow-up
• Contribute to a positive practice culture by mentoring colleagues and sharing best practices

Qualifications

• Current, unrestricted Physical Therapist license in Florida
• Active Basic Life Support (BLS) certification (American Heart Association or equivalent) preferred
• Comfortable working day shifts and able to provide coverage any day of the week, including weekends
• Strong clinical assessment and documentation skills
• Excellent communication and teamwork skills with patients, families, and staff
• Prior experience in post-acute, rehab, or outpatient settings is a plus but open to motivated clinicians

Education

• Doctor of Physical Therapy (DPT) preferred, or equivalent degree in Physical Therapy

Knowledge & Skills

• Skilled in therapeutic exercise prescription, manual techniques, and functional mobility training
• Knowledge of common diagnoses seen in post-acute rehabilitation and appropriate interventions
• Proficient with electronic medical records and accurate clinical documentation
• Ability to prioritize caseloads, manage time effectively, and adapt to changing patient needs
• Strong patient education and caregiver training capabilities
• Good problem-solving skills and clinical reasoning
