Contract Type: Travel Contract

Shift Type: Monday-Friday, Day shift (one of: 7:00 AM - 3:30 PM; 7:30 AM - 4:00 PM; 8:00 AM - 4:30 PM; 8:30 AM - 5:00 PM). Weekends required every other weekend (4 weekend days in a 4-week schedule). Day shift, with occasional floating within case management to meet facility needs.

Roles & Responsibilities

• Coordinate and manage patient transitions of care, ensuring safe, timely discharges
• Conduct utilization review using Milliman Guidelines to support appropriate care decisions
• Perform needs assessments and order DME as part of the plan of care
• Document thoroughly and accurately in Epic, including adherence to Epic Hardstop requirements
• Collaborate with interdisciplinary teams — social services, PT/OT, pharmacy and more — to remove barriers to discharge
• Provide care coordination across age groups from neonates to geriatrics
• Float within the case management service line as needed to cover gaps and maintain continuity
• Support regulatory compliance by following CMS and HIPAA guidelines in all case management activities

Qualifications

• AK RN license in hand required for auto-offer (screen for pending license; auto-offer requires license on file)
• Active BLS certification required
• Minimum 1 year of acute care experience (first-time travelers accepted)
• Epic EMR experience required (Epic Hardstop familiarity preferred)
• Utilization review / case management experience; ACM certification preferred
• Comfort working in an acute hospital environment (Magnet facility, Level II trauma)
• Willingness to cover every-other-weekend schedule and float within case management scope
• Candidates must reside at least 50 miles from the facility (locals not accepted)

Knowledge & Skills

• Proficient with Epic charting and workflows, including hardstop processes
• Knowledge of Milliman Guidelines for utilization review decisions
• Strong discharge planning and care coordination skills
• Experience with needs assessment and ordering DME
• Familiarity with CMS regulations and HIPAA compliance
• Comfort collaborating with interdisciplinary teams and using Microsoft Office/Teams
• Exposure to specialties such as Pediatrics, Women’s Services, ICU, Medical-Surgical, SDU/PCU/IMC/Obs is a plus
