Roles & Responsibilities

• Evaluate patients’ functional abilities and create individualized treatment plans that drive measurable progress
• Deliver skilled occupational therapy interventions to improve daily living, mobility, and independence
• Collaborate with interdisciplinary teams to ensure coordinated, person-centered care
• Document assessments, progress notes and discharge plans clearly and timely
• Educate patients and families on therapeutic strategies and adaptive techniques for home and community
• Monitor and adjust treatment plans based on outcomes and changing patient needs
• Advocate for patient safety and optimal recovery within the facility environment

Qualifications

• Current state license or certification as an Occupational Therapist (OT) — required
• COTA certification accepted for COTA-level roles (please indicate if applying as COTA)
• Current BLS/CPR certification or willingness to obtain upon hire
• Experience working in rehabilitation, outpatient or post-acute settings is highly valued
• Strong clinical reasoning, documentation skills, and the ability to work collaboratively
• Ability to pass background checks and meet facility onboarding requirements

Education

• Bachelor’s or Master’s degree in Occupational Therapy for OT applicants
• Associate degree or accredited OTA program for COTA applicants

Knowledge & Skills

• Skilled in ADL/IADL assessments and therapeutic activity planning
• Knowledge of adaptive equipment, splinting basics, and mobility techniques
• Proficient documentation skills and familiarity with electronic health records
• Excellent communication and patient/family education abilities
• Strong time management and caseload prioritization skills
• Team-oriented approach to interdisciplinary care
