Roles & Responsibilities

Evaluate patients and craft individualized physical therapy treatment plans tailored to functional goals
Deliver skilled therapy focused on mobility, strength, balance, and restoring independence
Monitor and document patient progress accurately and in a timely manner to guide care
Collaborate with interdisciplinary teams to support optimal patient outcomes and care transitions
Educate patients and caregivers on therapy goals and practical home exercise programs

Qualifications

Active California Physical Therapist license (required)
Experience in a Skilled Nursing Facility or rehabilitation setting preferred
Strong clinical judgment and excellent communication skills
Ability to work independently while contributing positively to a collaborative team

Knowledge & Skills

Solid command of physical therapy principles and patient-centered care
Skilled in mobility, balance, strength training, and functional task training
Clear and timely documentation practices
Comfortable working with interdisciplinary teams and educating patients/caregivers
Professional communication and clinical decision-making
