Roles & Responsibilities

• Assess patients' nutritional status and determine appropriate nutritional diagnoses
• Develop measurable, patient-centered nutrition care plans and coordinate implementation
• Provide nutrition education and counseling to patients, families, and caretakers
• Collaborate with interdisciplinary teams including nursing, pharmacy, and social services
• Review diet orders and address discrepancies in accordance with the diet manual
• Document assessments, interventions, education, and care plans in the medical record
• Perform unit rounds to identify patients at nutritional risk and follow up as needed
• Evaluate patient tolerance to therapeutic diets and adjust plans based on outcomes
• Assist in developing and monitoring clinical performance indicators for the department
• Maintain clear, timely communication with physicians regarding nutritional care concerns

Qualifications

• Acute care experience required — must-have for this role
• License/Certificate Verification required (state licensure as applicable)
• DCI/BVP certification listed in job requirements
• Completion of Prime Core Competencies Exam, Prime Skills Checklist, and Prime Specialty Exam
• Current required immunizations and screenings (COVID-19, TB, Hepatitis B, MMR, Varicella, Influenza, Tdap) and drug screen
• Strong documentation skills and comfort with medical record charting
• Excellent communication, cultural competence, and patient counseling skills
• Experience with therapeutic diets, diet order review, and nutrition education
