Roles & Responsibilities

• Provide compassionate, patient-centered care to assigned residents and patients
• Administer medications and document all treatments accurately and promptly
• Monitor vital signs, recognize changes in condition, and escalate concerns
• Collaborate with nurses, physicians and interdisciplinary team members for coordinated care
• Maintain thorough and accurate patient records in the medical chart
• Support patient mobility, hygiene and daily living activities with dignity and respect
• Participate in care planning and follow established clinical protocols and safety procedures

Qualifications

• Current, active LPN (Licensed Practical Nurse) license for New York state
• BLS/CPR certification (current) or willingness to obtain upon hire
• Previous hands-on experience in long-term care, skilled nursing, or a related setting is preferred
• Valid nursing credentials and ability to pass background check and health screenings
• Strong communication skills and a patient-first attitude

Education

• Graduation from an accredited practical/vocational nursing program (LPN/LVN diploma)

Knowledge & Skills

• Solid clinical assessment skills and medication administration knowledge
• Comfortable with wound care, basic treatments and monitoring patient conditions
• Familiarity with electronic health records and timely clinical documentation
• Ability to prioritize tasks, manage time and work effectively in a team
• Compassionate approach to patient care and strong interpersonal skills
