Roles & Responsibilities
- Provide compassionate bedside and in-home care to home health and hospice patients
- Administer medications and treatments according to plan of care and scope of practice
- Assess patient condition, monitor symptoms, and report changes to the care team
- Coordinate care with nurses, providers, and family caregivers to support holistic needs
- Document assessments, interventions and patient responses accurately and timely
- Educate patients and families on symptom management, medication use and safety
- Support end-of-life comfort measures and communicate respectfully with patients and families
- Participate in required competency testing and maintain current certifications
Qualifications
- Active LPN license (must be valid in Michigan or appropriate jurisdiction)
- BLS certification required
- Experience or interest in home health and hospice patient care
- Successful completion of Trinity Specialty Specific Competency Testing as required
- Respirator fit test and Trinity respirator evaluation completion where applicable
- Required health screening and immunizations: TB testing (2-step PPD or IGRA), MMR, Hepatitis B, Varicella, Tdap, Influenza, COVID-19 per facility policy
- Trinity 10 panel drug screen and license/certificate verification required
- Ability to travel to patient homes and work independently with strong communication skills

- Home health and hospice care principles and best practices
- Patient-centered assessment and symptom management
- Medication administration and basic wound care
- Infection control and immunization protocols
- Comfort with electronic documentation and timely charting
- Strong interpersonal skills for family-centered care and care coordination
- Ability to follow Trinity-specific competency checklists and testing procedures
