Roles & Responsibilities

• Assess patients in the home setting and create individualized occupational therapy plans that improve daily function
• Complete accurate, timely OASIS documentation and chart in Cerner to meet regulatory and quality standards
• Collaborate with PT, SLP, rehab aides, OTA/PTA and interdisciplinary team members to coordinate patient care
• Educate patients and caregivers on safe home strategies, adaptive equipment, and injury prevention
• Prioritize caseload and manage documentation to ensure high-quality, compliant visits
• Participate in orientation and mentor support to maintain consistent, excellent care across the region

Qualifications

• Active California Occupational Therapist license (state licensure required to submit)
• BLS (Basic Life Support) certification required
• Minimum 2 years of clinical experience (PT/OT/SLP experience accepted)
• Experience with OASIS documentation (required)
• Familiarity with Cerner or similar EMR systems (preferred)
• Home health experience preferred but not required
• Willingness to cover an assigned geographical region and work every 3rd weekend

Knowledge & Skills

• Proven ability to complete OASIS assessments and meet home health documentation standards
• Strong clinical assessment and treatment planning for orthopedics, stroke, spine and joint patients
• Skilled in patient and caregiver education for ADLs and safe home mobility
• Effective communication and teamwork with PT, SLP, aides and other providers
• Time management and caseload prioritization for efficient home visits
• Comfort working independently in patients’ homes and adapting treatment to varied environments
