Roles & Responsibilities

• Provide compassionate, high-quality nursing care to residents as a licensed practical nurse
• Administer medications and treatments accurately and document them thoroughly
• Monitor resident condition and report changes promptly to the interdisciplinary team
• Collaborate with RNs, physicians, and therapy staff to implement individualized care plans
• Assist with admissions, discharges, and transfers to ensure smooth transitions
• Educate residents and families about care plans and daily routines to promote comfort and understanding
• Maintain accurate, timely charting and follow facility policies and regulatory standards

Qualifications

• Current Licensed Practical Nurse (LPN) license — active and in good standing
• T-LPN classification indicated (as required by the facility)
• Comfortable working overnight shifts and providing weekend coverage every other week
• Ability to pass required background check and drug screen per facility policy
• Strong communication and teamwork skills, with a patient-centered attitude

Education

• Graduation from an accredited LPN/LVN program and possession of current state LPN licensure

Knowledge & Skills

• Solid fundamentals in medication administration and resident assessment
• Proficiency in documentation and electronic charting systems (facility-specific orientation provided)
• Skills in wound care, ADL assistance, and basic clinical procedures
• Comfortable working in long overnight shifts and prioritizing patient safety
• Good clinical judgment and the ability to escalate concerns appropriately
