Roles & Responsibilities

• Provide professional social work services appropriate for health services in the assigned setting
• Conduct assessments, develop care plans, and coordinate services that improve patient outcomes
• Collaborate with interdisciplinary teams (clinical, educational, and support staff) to support holistic patient care
• Advocate for patients and families to ensure access to community resources and needed supports
• Document interventions, progress, and discharge planning clearly and timely in patient records
• Participate in case conferences, team meetings, and communication with families and caregivers

Qualifications

• Licensed by the State of New Mexico Public Education Department (required)
• Area of specialty appropriate for health services (must be relevant to the clinical setting)
• Three years of successful work experience in the medical field is desirable/preferred
• Ability to work five 8-hour shifts (40 hours/week) for the duration of the 13-week contract
• Strong communication, documentation, and care-coordination abilities
