Roles & Responsibilities

• Provide compassionate, patient-centered hospice nursing care across settings (private homes, nursing homes, assisted living)
• Conduct thorough patient assessments and develop individualized care plans focused on symptom management and comfort
• Serve as the primary RN for an assigned caseload, coordinating and leading the interdisciplinary team
• Educate and coach patients and family caregivers to build confidence in day-to-day care
• Use critical thinking to manage complex symptoms and make timely clinical decisions
• Document care accurately and communicate effectively with the care team and families
• Provide occasional on-call support and weekend coverage as part of team backup

Qualifications

• Active Registered Nurse (RN) license—ability to practice in Colorado preferred
• Strong clinical assessment skills and experience in symptom management; hospice or palliative care experience is a strong plus
• Excellent communication and teaching abilities to support patients and family caregivers
• Demonstrated leadership in coordinating care and working within an interdisciplinary team
• Current BLS (Basic Life Support) certification or willingness to obtain
• Comfort providing care in a variety of settings including private homes, long-term care, and assisted living

Education

• Associate Degree in Nursing (ADN) or Bachelor of Science in Nursing (BSN) preferred
