Roles & Responsibilities

• Provide compassionate, hands-on physical therapy care to long-term care and SNF residents
• Implement established treatment plans under the direction of the supervising Physical Therapist
• Monitor and document patient progress, adjusting care details and communicating outcomes
• Assist with mobility training, transfers, gait training, and therapeutic exercises
• Collaborate with nursing and interdisciplinary team members to coordinate safe, patient-centered care
• Educate residents and families on home programs, safety strategies, and functional goals
• Maintain accurate, timely documentation and comply with facility policies and regulatory standards

Qualifications

• Active or eligible Physical Therapy Assistant (PTA) license for California
• Current BLS/CPR certification (or willing to obtain upon hire)
• Experience in skilled nursing facility (SNF) or long-term care settings preferred
• Comfort working with complex medical populations and progressive mobility needs
• Strong communication skills and the ability to collaborate with multidisciplinary teams
• Ability to pass background checks and meet facility immunization / screening requirements

Education

• Associate degree from an accredited Physical Therapist Assistant program (or equivalent)

Knowledge & Skills

• Solid working knowledge of therapeutic exercise, transfers, gait training and ADL support
• Familiarity with long-term care goals, restorative programs, and SNF documentation
• Skilled in safe patient handling and use of assistive devices
• Proficient in clinical documentation and outcomes tracking
• Good clinical judgment, time management, and interpersonal skills
