Roles & Responsibilities

Deliver personalized physical therapy to patients across four rural counties
Conduct comprehensive Oasis Assessments and Start of Care evaluations
Help patients recover from falls, fractures, and other common elderly injuries
Track visits and time using a tablet for seamless documentation
Build trust with patients while providing care in their homes and communities

Qualifications

Current Physical Therapist license in Indiana
Experience with Oasis Assessments and Start of Care documentation
Comfortable working independently in rural settings
Strong communication and time-management skills
