Roles & Responsibilities

• Receive and give clear, accurate report on each resident’s condition to ensure continuity of care
• Make routine rounds and assess new residents or anyone who is ill, has fallen or sustained an injury
• Document assessments and care activities according to facility policy
• Notify nursing staff promptly of any changes in residents’ conditions or needs
• Provide direct patient care and respond to call lights quickly and compassionately
• Support residents’ families with information and a respectful bedside manner
• Collaborate with nursing and other departments to promote a positive care environment
• Contribute to effective shift handoffs and reporting to maintain high-quality care

Qualifications

• Current California CNA license required
• Certification as a Certified Nursing Assistant (CNA)
• Experience or comfort working in a Skilled Nursing Facility (SNF) environment
• Willingness to float to other units when needed
• Availability to work some weekends and holidays
• Strong communication skills and a team-player attitude

• Resident assessment and recognizing changes in condition
• Accurate documentation and reporting/shift handoff practices
• Effective communication with residents, families and interdisciplinary teams
• Teamwork and collaboration within a clinical setting
