Roles & Responsibilities

• Provide direct patient care and respond promptly to call lights to ensure resident comfort and safety
• Conduct resident assessments, make rounds, and promptly report any changes in condition
• Document assessments and care per facility policy to maintain accurate resident records
• Communicate clearly and give/receive reports to ensure continuity of care
• Collaborate with nurses, families and other departments to promote resident-centered care
• Float to other units as needed and support team needs during weekends and holidays
• Promote positive relationships with residents, families and healthcare team members

Qualifications

• California CNA License required — must have active, valid license
• Certified Nursing Assistant (CNA) credential
• Ability to float to different units as needed
• Availability for weekends and holidays
• Strong communication skills and a team-player attitude
• Previous experience in a skilled nursing facility or long-term care setting is a plus

• Resident assessment and monitoring

• Accurate documentation and reporting
• Clear communication with nursing staff, residents and families
• Teamwork and the ability to collaborate across departments
