Roles & Responsibilities

• Assess residents' functional abilities and create person-centered treatment plans
• Implement evidence-based therapeutic interventions to improve mobility and independence
• Document progress accurately and timely according to facility policies and regulatory standards
• Collaborate with nursing, SLP, OT and interdisciplinary team members to coordinate care
• Educate residents and families on safe mobility strategies, fall prevention and exercise programs
• Maintain productivity standards and contribute positively to the therapy team culture
• Participate in team meetings and contribute to quality improvement initiatives

Qualifications

• Current New Mexico Physical Therapist license (or eligible to obtain prior to start)
• Doctor of Physical Therapy (DPT) preferred — PT licensure and clinical competency required
• 1+ years of clinical experience; SNF/long-term care experience is a strong plus
• Current CPR/BLS certification
• Comfort with documentation systems, productivity expectations (90% productivity requirement)
• Professional presentation (facility dress code: black scrubs) and reliable onsite presence

Education

• Doctor of Physical Therapy (DPT) preferred or accredited PT degree with state licensure

Knowledge & Skills

• Skilled in physical therapy evaluation, treatment planning and progress measurement
• Experience with geriatric and skilled nursing populations, including fall risk management
• Strong clinical reasoning, manual therapy and therapeutic exercise skills
• Effective interdisciplinary communication and patient/family education abilities
• Familiarity with SNF regulatory requirements and documentation standards
• Time management and organizational skills to meet productivity targets
