Roles & Responsibilities

• Coordinate and deliver individualized physical therapy services that restore function and promote independence
• Assess patients, develop and revise plan(s) of care, and track progress toward meaningful goals
• Teach rehabilitation skills to patients and caregivers so gains last beyond therapy sessions
• Apply therapeutic modalities such as ultrasound, electrical muscle stimulation, and whirlpool when clinically appropriate
• Document skilled and medically necessary care thoroughly, timely, and in compliance with Medicare, JCAHO, and payer requirements
• Perform PTA and Home Health Aide supervisory visits and provide clear written instructions per policy
• Communicate significant clinical changes to physicians and the care team to ensure safe, coordinated care
• Participate in quality improvement activities, staff meetings, and community-facing initiatives to strengthen services

Qualifications

• Licensed Physical Therapist (or eligible for state licensure) — ability to practice in South Dakota preferred
• Experience providing home health or outpatient physical therapy services preferred
• Comfortable using and documenting physical modalities: ultrasound, electrical muscle stimulation, whirlpool
• Experience supervising Physical Therapy Assistants and Home Health Aides (or willingness to supervise per federal/state rules)
• Familiarity with Medicare, JCAHO, and payer documentation/compliance expectations
• Strong communication skills and a commitment to excellent patient-centered customer service

Education

• Doctor of Physical Therapy (DPT) preferred; Master's in Physical Therapy considered
• Professional education/credentials aligned with state licensure requirements

• Rehabilitation techniques and therapeutic exercise planning
• Patient assessment and plan-of-care development
• Clinical documentation best practices and productivity standards
• Patient education and safety-focused care planning
