Roles & Responsibilities

• Provide skilled physical therapy evaluations and treatment in patients' homes to improve mobility and function
• Develop individualized treatment plans focused on meaningful patient goals and safety in the home environment
• Perform thorough home safety and mobility assessments and recommend adaptive equipment when appropriate
• Document care accurately and promptly (Epic preferred) and complete required competency checklists
• Educate patients and caregivers on exercise programs, fall prevention, and safe functional techniques
• Collaborate with interdisciplinary team members to coordinate care and optimize patient outcomes
• Maintain compliance with required screenings, certifications, and facility policies

Qualifications

• Current Physical Therapist license or ability to obtain licensure in Iowa (license/certificate verification required)
• Basic Life Support (BLS) certification required
• Home health experience preferred
• Epic experience preferred but not required
• Willingness to complete Trinity Specialty Specific Competency Testing and Trinity Skills Checklist
• Ability to meet required health screenings and immunizations as listed (drug screen, TB testing, MMR, COVID-19, etc.)

Knowledge & Skills

• Skilled in patient assessment, treatment planning, and therapeutic exercise
• Knowledge of home safety evaluation and fall prevention strategies
• Proficient documentation practices; Epic experience is a plus
• Strong communication and patient/caregiver education skills
• Ability to work independently in community/home settings and manage a patient caseload
