Roles & Responsibilities

• Provide skilled physical therapy interventions and carry out treatment plans designed to restore mobility and function
• Assist with gait training, therapeutic exercise and use of adaptive equipment to promote resident independence
• Document patient progress accurately and in a timely manner to support continuity of care
• Collaborate with nurses, therapists and interdisciplinary team members to create meaningful rehab goals
• Educate residents and families on home programs, safety and fall prevention to support long-term outcomes
• Maintain a safe, compassionate environment while adhering to facility policies and regulatory standards

Qualifications

• PTA credential or completion of an accredited PTA program preferred; open to new grads as noted in the order
• Comfort working in a skilled nursing facility environment
• Strong communication skills and a team-oriented mindset
• Current basic life support (BLS) as required by facility policies (obtainable if needed)
• Ability to pass required background checks and facility onboarding

Education

• Graduation from an accredited Physical Therapist Assistant program preferred

Knowledge & Skills

• Patient assessment and basic mobility evaluation skills
• Hands-on therapeutic modalities, exercise prescription and gait training
• Skilled documentation and familiarity with clinical charting
• Knowledge of resident safety, fall prevention and safe transfers
• Good interpersonal skills for working with residents, families and the care team
