Roles & Responsibilities

• Assist patients with activities of daily living, providing respectful and compassionate support
• Monitor and record vital signs and report changes to the nursing team promptly
• Support safe patient transfers and mobility, using proper body mechanics
• Maintain a clean, safe, and organized patient environment
• Communicate clearly with patients, families, and interdisciplinary staff to promote coordinated care
• Follow infection control protocols and facility policies to protect patients and coworkers
• Document care activities accurately in the patient record and during handoffs

Qualifications

• Current Patient Care Technician (PCT) certificate or Certified Nursing Assistant (CNA) credential preferred
• Basic Life Support (BLS/CPR) certification required or obtainable upon hire
• Recent hands-on experience in long-term care, skilled nursing, or acute settings is a plus
• Up-to-date immunizations and ability to pass background and health screenings
• Strong communication skills, reliability, and a patient-centered attitude

Education

• High school diploma or GED required; PCT/CNA training certificate preferred

Knowledge & Skills

• Proficient in basic patient care tasks and safety procedures
• Ability to take and document accurate vital signs
• Familiarity with infection control and standard precautions
• Comfortable assisting with mobility, transfers, and ADLs
• Team-oriented mindset and effective interpersonal skills
• Basic computer skills for electronic charting and documentation
