Roles & Responsibilities

• Assess patients in the home setting to identify functional needs and goals
• Develop and implement individualized occupational therapy plans of care
• Provide hands-on treatment, therapeutic interventions, and caregiver education
• Complete OASIS assessments and visit documentation accurately and timely
• Coordinate care with interdisciplinary team members and physicians
• Manage a visit-based caseload while meeting productivity/point expectations
• Travel safely within assigned territories to deliver high-quality home health services

Qualifications

• Current California OT license (required)
• BLS (required)
• Valid driver’s license, current auto insurance, and vehicle registration
• Up-to-date immunizations and screenings: TB (annual), MMR, Varicella, HepB, TDap (within 10 years), Flu (annual)
• Drug screen and recent physical (as required)
• Comfort treating COVID+ and PUI patients (required for some assignments)
• Home health experience preferred but new grads are welcome
• Willingness to work weekends and travel between service areas when needed

Education

• Occupational Therapy degree (OT, MSOT, or OTD). New grads welcome.

Knowledge & Skills

• Proficiency with OASIS assessments and home health documentation
• Strong patient documentation and visit-based care skills
• Ability to convert visit types into productivity/point systems
• Excellent time management and clinical decision-making in the home environment
• Clear communication and collaboration skills with patients, families, and clinicians
• Comfort using EMR systems and completing timely charting
