Shift Type: 8-hour shift; specific start and end times and days to be determined with facility (Total Hours: 8)

Hours per Week: 40

Candidate Pay Rate: $51 per hour

Roles & Responsibilities

Provide direct patient care with compassion and attention to detail
Administer medications and treatments accurately and safely
Perform routine assessments and report changes in patient condition
Document care in the chart thoroughly and timely
Collaborate with the care team to support individualized care plans
Assist with patient comfort, mobility, and activities of daily living
Support a positive, respectful environment for residents and families

Qualifications

Active LPN license; Pennsylvania license preferred or eligible
Current BLS/CPR certification or ability to obtain upon hire
Comfortable with medication administration and basic clinical procedures
Strong communication skills and a team-first attitude
Previous long-term care or skilled nursing experience is a plus
Ability to pass required background checks and health screenings

Education

LPN diploma or equivalent from an accredited program

Knowledge & Skills

Solid understanding of nursing fundamentals and standards of care
Medication administration and documentation best practices
Basic assessment skills for monitoring patient status
Familiarity with electronic health records and charting
Good time management and prioritization in a busy setting
Compassionate bedside manner and strong interpersonal skills
