Roles & Responsibilities
- Provide compassionate, direct patient care with attention to dignity and comfort
- Administer medications and treatments accurately and document appropriately
- Perform routine assessments and report changes in patient condition promptly
- Collaborate with nursing staff and interdisciplinary team to coordinate care
- Maintain clear, timely charting and ensure regulatory compliance
- Support patient mobility, activities of daily living, and safe care transitions
Qualifications
- Active LPN license in Pennsylvania (required)
- Current BLS/CPR certification (or willingness to obtain upon hire)
- Ability to pass background check and meet facility immunization requirements
- Comfortable working in a team-oriented long-term care environment
- Previous LPN experience preferred but new grads with strong clinical skills are encouraged to apply
Education
- Graduate of an accredited practical/vocational nursing program
Knowledge & Skills
- Medication administration and basic nursing procedures
- Patient assessment and documentation best practices
- Infection control and safety protocols
- Strong communication and teamwork skills
- Basic familiarity with electronic health records and charting
