Contract Type: Per-Diem

Shift Type: 8-hour shift — exact start/end times and specific days to be confirmed by the facility (schedule set by Autumn Lake Healthcare)

Hours per Week: 40

Candidate Pay Rate: $36 per hour

Roles & Responsibilities

Provide compassionate basic nursing care and assist residents with activities of daily living
Administer medications and treatments as directed and document accurately
Monitor vital signs and report changes in patient condition to the care team
Maintain clear, timely documentation in the resident chart and electronic health records
Collaborate closely with RNs, CNAs, and interdisciplinary staff to deliver excellent care
Follow infection control protocols and facility policies to keep residents safe
Communicate respectfully with residents and families to support person-centered care
Participate in shift handoffs and contribute to a positive team culture

Qualifications

Current LPN license in Connecticut (required)
Graduation from an accredited practical nursing program
Basic Life Support (BLS) certification preferred or willing to obtain
Previous experience in long-term care or similar setting is a plus
Strong communication skills and the ability to work well in a team

Education

LPN diploma or certificate from an accredited practical nursing program

Knowledge & Skills

Medication administration and documentation
Patient assessment and monitoring
Familiarity with infection control practices
Basic electronic health record navigation
Time management and prioritization during shifts
Compassionate resident-centered care and communication
