Roles & Responsibilities
- Provide direct, compassionate patient care in line with facility standards
- Administer medications and treatments safely and accurately
- Monitor patient status and communicate changes to the care team
- Maintain clear, timely documentation of care and observations
- Collaborate with nurses, clinicians, and support staff to optimize outcomes
- Support patient comfort, dignity, and daily living needs
Qualifications
- Current LPN credential (Licensed Practical Nurse)
- Ability to practice in Pennsylvania preferred given facility location
- Strong communication and teamwork skills
- Comfortable with medication administration, basic assessments, and documentation
Knowledge & Skills
- Foundational nursing skills and basic clinical assessments
- Safe medication administration and adherence to protocols
- Accurate charting and electronic documentation
- Effective communication with patients, families, and clinical teams
- Time management and the ability to prioritize patient needs
