Candidate Pay Rate: $34 per hour

Roles & Responsibilities

Provide compassionate, hands-on nursing care to residents in accordance with plan of care
Administer medications and treatments safely and accurately
Monitor resident condition and promptly report changes to the care team
Document assessments, interventions, and outcomes in the medical record clearly
Collaborate with nursing and interdisciplinary staff to support resident goals
Educate residents and families about care plans and promote resident dignity

Qualifications

Current Licensed Practical Nurse (LPN) license (Pennsylvania) required
Hands-on patient care experience preferred; experience in long-term care is a plus
Current basic competency in medication administration and documentation
Strong communication and teamwork skills
Ability to pass required background checks and health screenings

Education

Completion of an accredited LPN/Vocational Nursing program

Knowledge & Skills

Skilled in direct patient care and monitoring
Knowledge of medication administration and safe practices
Competent with clinical documentation and charting
Able to prioritize tasks and respond to changing resident needs
Strong interpersonal skills for working with residents, families, and staff
