Roles & Responsibilities
- Provide compassionate, patient-centered nursing care to residents
- Administer medications and document treatments accurately
- Monitor resident vitals and report changes to the care team
- Collaborate with RNs, CNAs, and interdisciplinary staff to deliver coordinated care
- Maintain clear and timely clinical documentation
- Support resident activities of daily living and promote comfort and dignity
- Participate in care planning and follow established facility protocols
Qualifications
- Current and active LPN license (Pennsylvania preferred)
- Completion of an accredited practical nursing program
- Basic Life Support (BLS) or CPR certification
- Strong communication skills and a team oriented attitude
- Previous experience in long term care or similar settings is a plus
Education
- Completion of an accredited LPN program
- High school diploma or GED
Knowledge & Skills
- Medication administration and charting best practices
- Patient assessment and basic clinical monitoring
- Infection control and standard safety procedures
- Good time management and ability to prioritize tasks
- Empathy, strong interpersonal skills, and resident advocacy
