Candidate Pay Rate: $34 per hour

Roles & Responsibilities

Provide compassionate, direct patient care and support activities of daily living
Administer medications and treatments safely and accurately
Monitor vital signs and promptly report changes in patient condition
Document patient care thoroughly and professionally in the facility chart/EMR
Collaborate with nurses, clinicians, and support staff to deliver coordinated care
Assist with admissions, discharges, and patient comfort measures
Support a positive, patient-centered environment through clear communication

Qualifications

Valid LPN (Licensed Practical Nurse) license in Pennsylvania or eligible for PA licensure
Current CPR/BLS certification (or willingness to obtain upon hire)
Comfortable with basic medication administration and wound care
Strong communication and teamwork skills; a caring bedside manner
Ability to pass background checks and meet facility health requirements
Previous experience in long-term care or skilled nursing preferred but open to motivated clinicians

Education

Graduate of an accredited LPN/Vocational Nursing program
High school diploma or GED required

Knowledge & Skills

Patient assessment and basic clinical monitoring
Medication administration and documentation best practices
Familiarity with electronic medical records (EMR) and charting
Infection control procedures and safe work practices
Time management and prioritization in a fast-paced care setting
Effective communication with patients, families, and the care team
