Roles & Responsibilities

Provide compassionate direct patient care with attention to dignity and safety
Administer medications and treatments accurately and document appropriately
Perform routine assessments and report changes in patient condition promptly
Collaborate with nurses, therapists, and the care team to create positive outcomes
Maintain accurate, timely charting in the patient record and follow facility protocols
Support residents with activities of daily living while promoting independence

Qualifications

Valid LPN (Licensed Practical Nurse) license in Pennsylvania required
Current Basic Life Support (BLS) certification preferred or willing to obtain
Previous experience in skilled nursing or long-term care is a plus
Ability to pass background checks and meet facility health requirements
Strong communication skills and a patient-centered approach

Education

Practical Nursing diploma or equivalent
Active LPN license (Pennsylvania)

Knowledge & Skills

Skilled in medication administration and basic clinical procedures
Proficient with patient documentation and electronic medical records
Knowledge of infection control and safety best practices
Good assessment skills and ability to escalate concerns appropriately
Team-oriented with strong interpersonal and organizational skills
Comfortable working in a fast-paced long-term care environment
