Roles & Responsibilities

Provide compassionate, hands-on nursing care to residents in a skilled nursing setting
Administer medications and treatments accurately and document in the medical record
Monitor resident condition and report changes promptly to the interdisciplinary team
Assist with activities of daily living while promoting independence and dignity
Collaborate with nurses, therapists, and caregivers to deliver coordinated care plans
Support resident and family education with empathy and clear communication
Uphold safety, infection control, and regulatory standards at all times

Qualifications

Current, active LPN license in Pennsylvania (required)
Completion of an accredited LPN program
Basic Life Support (BLS/CPR) certification preferred or willingness to obtain
Demonstrated ability to provide compassionate care in a long-term or skilled nursing setting
Strong communication skills and ability to work collaboratively on a care team
Reliable, professional, and committed to resident-centered care

Education

Graduation from an accredited LPN/Vocational Nursing program
High school diploma or equivalent

Knowledge & Skills

Medication administration and documentation accuracy
Wound care fundamentals and basic assessment skills
Familiarity with electronic medical records and proper charting
Infection prevention, safety protocols, and regulatory compliance
Time management and prioritization in a fast-paced care environment
Effective interpersonal skills with residents, families, and teammates
