Roles & Responsibilities

Provide compassionate direct patient care and support residents with daily living activities
Administer medications and document care accurately according to policy
Monitor and record vital signs, reporting any changes to the care team promptly
Collaborate with RNs, CNAs, and interdisciplinary staff to deliver coordinated care
Maintain accurate charts and patient records, ensuring regulatory compliance
Support a safe, clean, and respectful environment for residents and families

Qualifications

Active LPN license in Pennsylvania (must have or be eligible)
Current BLS/CPR certification or willingness to obtain
Comfort working 8-hour shifts and performing hands-on patient care
Strong communication skills and a patient-focused attitude

Education

LPN diploma or practical nursing certificate from an accredited program

Knowledge & Skills

Medication administration and basic clinical assessments
Infection control practices and safety procedures
Patient documentation and basic electronic charting familiarity
Ability to prioritize, multitask, and work collaboratively
