Roles & Responsibilities

• Provide compassionate, patient-centered care during night shifts to ensure safety and comfort
• Administer medications and treatments accurately and document in the patient record
• Monitor and record vital signs, recognize changes in condition, and escalate concerns promptly
• Collaborate with nursing colleagues and interdisciplinary teams to coordinate effective care plans
• Perform routine assessments and assist with activities of daily living to promote recovery
• Follow infection control protocols and maintain a safe, clean environment
• Communicate clearly with patients, families, and staff to provide continuity of care

Qualifications

• Active LPN license in West Virginia (required)
• Completion of an accredited practical nursing program (diploma or certificate)
• Current BLS/CPR certification (required)
• Immunizations and TB screening up to date per facility policy
• Successful background check and drug screen per facility requirements
• Comfortable working 12-hour night shifts and able to manage patient care independently
• Previous experience in long-term care or acute care settings is a plus

Education

• Practical Nursing diploma or equivalent (completion of accredited LPN program) and current state licensure

Knowledge & Skills

• Medication administration and accurate charting
• Patient assessment and monitoring of clinical changes
• Familiarity with electronic medical records and documentation best practices
• Infection prevention and control procedures
• Strong communication, teamwork, and time-management skills
• Basic wound care and support with ADLs
