Roles & Responsibilities

• Provide compassionate, hands-on nursing care to residents and patients
• Administer medications and treatments accurately and safely
• Monitor vital signs and report changes in condition promptly
• Document care and observations thoroughly and in a timely manner
• Collaborate with nurses, therapists, and care team members to implement care plans
• Assist with activities of daily living and promote patient dignity and comfort
• Support infection control and safety practices to protect patients and staff
• Participate in shift handoffs and update the team on patient status

Qualifications

• Valid LPN license in Pennsylvania (or eligibility to obtain one)
• Current BLS/CPR certification (or willingness to obtain upon hire)
• Comfortable administering medications and following established care plans
• Strong communication skills and a team-oriented approach
• Compassionate bedside manner and respect for patient dignity
• Previous experience in a long-term care, skilled nursing, or similar setting is a plus

Education

• Graduate of an accredited Practical/Vocational Nursing program
• LPN/VN diploma or certificate required

Knowledge & Skills

• Skilled in medication administration and basic nursing procedures
• Knowledge of infection control and patient safety practices
• Ability to monitor, assess, and report changes in patient condition
• Proficiency in clear, concise documentation
• Strong interpersonal skills for working with patients, families, and the care team
• Time management and ability to prioritize patient needs during shifts
