Candidate Pay Rate: $34 per hour

Roles & Responsibilities

Provide compassionate, person-centered nursing care to residents in a long-term care setting
Administer medications and treatments safely and accurately according to policy
Monitor residents' conditions, record vital signs, and document care in the chart
Collaborate with nurses, therapists, and support staff to ensure coordinated care
Educate residents and families about care plans and clinical changes in an approachable way
Maintain accurate, timely documentation and communicate changes to the care team

Qualifications

Valid LPN (Licensed Practical Nurse) license in Pennsylvania required
Current BLS/CPR certification required or obtained upon hire
Graduate of an accredited Practical Nursing program
Comfortable working in a long-term care or skilled nursing environment; prior LTC experience is a plus
Strong communication skills and the ability to work effectively on a team

Education

Graduate of an accredited Practical Nursing (LPN) program

Knowledge & Skills

Medication administration and documentation best practices
Basic wound care and catheter care knowledge
Proficiency taking and recording vital signs accurately
Familiarity with electronic health record systems preferred
Ability to prioritize tasks, multitask, and remain calm under pressure
