Roles & Responsibilities

• Provide compassionate, patient-centered nursing care consistent with LPN scope of practice
• Administer medications and document treatments accurately and timely
• Monitor and record patient vital signs, symptoms and responses to interventions
• Collaborate with RNs, providers and interdisciplinary team members to support care plans
• Assist with admissions, discharges, and safe patient transfers
• Maintain clear, professional documentation in the patient chart/EHR
• Adhere to infection control policies, safety standards and facility protocols
• Support patient and family education with empathy and clear communication

Qualifications

• Active LPN license in New Hampshire (required)
• Completion of an accredited LPN/LVN program
• Current BLS/CPR certification (or willingness to obtain)
• Ability to work per-diem shifts and adapt to variable schedules
• Strong clinical judgment, attention to detail, and dependable work ethic
• Excellent communication and teamwork skills

Education

• Graduate of an accredited LPN/LVN program
• High school diploma or equivalent

Knowledge & Skills

• Competence in medication administration and basic nursing procedures
• Skilled at taking and interpreting vital signs and patient assessments
• Familiarity with basic wound care and ADL assistance
• Comfortable with electronic health record documentation
• Good time management, prioritization and patient advocacy abilities
• Professional bedside manner and effective family communication
