Roles & Responsibilities

• Provide compassionate direct patient care with attention to safety and comfort
• Administer medications and treatments in accordance with facility policy
• Monitor and document patient condition, reporting changes to the care team
• Assist patients with activities of daily living and mobility needs
• Collaborate with RNs, providers, and interdisciplinary staff to support care plans
• Maintain accurate, timely charting in the electronic health record
• Adhere to infection control standards and facility policies for safety
• Participate in quality improvement initiatives to enhance patient outcomes

Qualifications

• Active LPN license required
• Current BLS/CPR certification or willingness to obtain upon hire
• Completion of an accredited practical nursing program
• Ability to pass background check and drug screen as required
• Strong communication skills and ability to work well on a care team
• Comfortable using electronic health record systems for documentation

Education

• Diploma or certificate in Practical Nursing from an accredited program

Knowledge & Skills

• Medication administration and documentation
• Basic wound care and dressing changes
• Accurate vital signs assessment and patient monitoring
• Patient education and discharge support
• Care planning and prioritization in a fast-paced environment
• Time management and the ability to multitask effectively
• Understanding of infection prevention and control practices
